Talk With a Care Strategist
For Patients

A call from your doctor’s care team.

MatterCCM works with doctors’ offices to support their patients between visits. If we have called you, it was on behalf of your doctor. This page explains how to check that a call is genuine, what to expect, what it may cost, and how to stop at any time.

This is not an emergency service. If you have a medical emergency, call 911.

Is This Call Genuine?

How to check a call came from us.

We only call patients whose doctor’s office works with us, and we always say which office we are calling for. If you are not sure, you do not have to stay on the line.

  1. Hang up and call your doctor’s office on the number you already have for them. Ask whether they work with MatterCCM.
  2. Or call our patient line and give the name of your doctor’s office. We will confirm whether you are on their program.

We will never:

  • Ask for your bank account or credit card details
  • Ask you to pay anything over the phone
  • Ask for your Social Security number
  • Pressure you to join, or to decide on the first call

If someone does any of these while saying they are from MatterCCM, it is not us. Hang up and call your doctor’s office.

What to Expect

Support between your doctor’s visits.

Your doctor’s office offers this program to patients living with ongoing health conditions. It means someone from your care team checks in with you regularly, so problems are noticed early and you are not left on your own between appointments.

  1. You are invited

Your doctor’s office, or we on their behalf, will contact you to explain the program. Joining is your choice.

  2. You agree before anything starts

We explain what the program involves and what it may cost, and ask for your permission. Nothing begins until you say yes.

  3. A first conversation

A member of your care team talks with you about your health conditions, your medicines and what matters to you, and helps put together your care plan.

  4. Regular check-ins

Each month, someone from your care team calls to see how you are doing: your medicines, any new symptoms, upcoming appointments, and anything you need help arranging.

  5. Your doctor stays in charge

We share notes with your doctor’s office after each contact. Your doctor makes all decisions about your medical care.

If your doctor has also given you a device such as a blood pressure monitor, its readings reach your care team automatically. If a reading needs attention, someone will follow up with you.

Cost

What it may cost you.

Care management is a Medicare Part B service. Like most Part B services, you may owe a coinsurance — usually 20% of the Medicare-approved amount — once you have met your Part B deductible.

Many supplemental plans (Medigap) and Medicaid cover some or all of this. Your doctor’s office bills for the program, not MatterCCM, and you will be told about any cost before you agree to join.

Medicare allows only one doctor’s office to provide this service to you in a given month. If another office already does, let us know.

Stopping

You can stop at any time.

Tell any member of your care team, call our patient line, or call your doctor’s office. You do not need to give a reason.

Under Medicare’s rules, stopping takes effect at the end of the current calendar month. Leaving the program does not change any other care you receive from your doctor.

Your Privacy

Your information is protected.

Your health information is protected by federal privacy law (HIPAA). We use it only to provide the care your doctor’s office has arranged, and we share it only with your care team.
